Abstract Background: Gallbladder torsion is an exceptionally rare but pottentially life threating condition that often mimics acute cholecystitis. Case presentation: This case study presents a 90 year old woman with cognitive impairment who was diagnosed intraoperitevely with gallbladder torsion during emergency laparotomy Conclusion: The case highlights the diagnostic challenges posed by nonspecific symptoms and limitations in patient communication. Prompt surgical intervention led to a favorable outcome, emphasizing the importance of considering gallbladder torsion in the differential diagnosis of acute abdomen in elderly patients.
